Students, faculty, staff, and other members of the local community can apply for a US passport book and/or passport card directly through FSU's Passport Acceptance Facility in the International Programs office. The FSU Passport Acceptance Facility is appointment only; walk-ins cannot be accommodated. You don't need to be affiliated with FSU to make and appointment.
The FSU Passport office can provide you with the forms you'll need to apply for a US passport. You can also get a passport photo taken-even photos for passports or visas of many other countries. You need an appointment even if you only need photos.
As a Passport Acceptance Facility, our office is authorized to receive your completed application for a US passport and submit the application along with all necessary documentation to US Passport Services. Passport Services will then determine eligibility for a US Passport. It is important to note that only the US Department of State can make the final decision to issue a passport to an individual.

Appointment Information
The FSU Passport Acceptance Facility is open Monday through Friday by appointment only.
Appointments open no more than three weeks in advance if there a no available appointments at this time, check back the next business day.

- Appointments must be scheduled at least 24 hours prior, and appointments will only become available three weeks in advance.
- If you do not arrive within 10 minutes after the start of your appointment, your appointment will be canceled.
- Families can be scheduled as one appointment, please reference the appointment types below.
- Prior to coming in at your scheduled appointment time, it is important to review the procedures sent in your appointment confirmation email.
Appointment Types
See below for the different types of appointment bookings. Appointments will automatically factor in photo services, you do NOT need to book a separate "photo only" appointment.
Adult (16+) Passport Appointment

- If you have small children under the age of 5, you may want to consider booking a large family appointment.

Please ensure you book the correct appointment type. If the wrong appointment type is booked, your appointment may not be honored.
If you need to apply for 7 or more applicants at once or if you anticipate requiring more than the allotted appointment time, please email IP-Passports@fsu.edu for scheduling.
Processing Times
Routine Processing: 4-6 weeks
Expedited Processing: 2-3 weeks (Additional fee required)
Note that passport processing times are subject to change. Refer to the U.S. Department of State website for the most up-to-date processing times.
Shipping: It may take up to 2 weeks for applications to arrive by mail at a passport agency or center from the FSU Passport Office, and up to 2 weeks for you to receive a completed passport in the mail after it is printed. Expedited shipping options are available for a fee (see "Fees" section for details).
Requirements
Note: Please be aware of fraudulent websites claiming to offer passport services. There are official-looking websites that offer to issue or renew U.S. passports with the purpose of scamming applicants. Some websites are private companies that are not associated with the U.S. Department of State and will charge you for services that are available to you for free. Others are scammers trying to take your money and personal information. Never pay U.S. Passport Service fees online unless you are renewing online via Travel.State.Gov only. This is the only official government website for passport services.
All Applicants:
- Fill Out Form DS-11 “Application for U.S. Passport,” which is used to apply for a passport book and/or card. You have the option to utilize the provided link to fill out the electronic version of the DS-11 form. After completing it, you can either print the form and bring it to your appointment, or you may choose to handwrite your application at the time of your appointment.
- A passport book is a small booklet that can be used for international travel by land, sea, or air. It is the only document approved for international air travel.
- A passport card is a cheaper alternative to the passport book and can only be used for travel to Canada, Mexico, Bermuda, and Caribbean countries by land or sea. The passport card cannot be used for international air travel.
- Submit Evidence of US Citizenship, your proof of citizenship must be an original or certified copy, photocopies are not accepted. The following documents can be used as proof of citizenship:
- Expired passport book or card
- Birth Certificate
- Must be an original or certified copy.
- The US Department of State also requires long-form birth certificates (i.e. must include parental information).
- Consular Report of Birth Abroad
- Certificate of Naturalization
- Birth Certificate
- Certificate of Citizenship
- Photo Identification: Review this list of acceptable forms of identification.
- Passport Photo
- May be taken in our office at the time of your application for an additional $10.74 fee.
- Applicable fees: Payment includes two separate fees; one to the Department of State, and one to Florida State University. Check or Money Order is required for the Department of State fee, starter checks are not accepted. Please select the "Fees" section for more information.
Applicants Age 16 or 17:
Applicants ages 16 and 17 qualify as an adult for passport application purposes. However, they are required to demonstrate parental awareness at the time of their appointment.
How to Show Parental Awareness:
- A legal parent or legal guardian appearing with you in person when you apply for your passport. The agent or employee accepting the application will ask your legal parent or legal guardian to sign Form DS-11. Bring a photocopy of that parent or guardian's ID.
- Submit a signed note from your legal parent or legal guardian with a photocopy of their ID.
- Submit proof that your legal parent or legal guardian is paying your application fees (Example: parent or guardian name is written on the check or money order).
- We may ask you to submit a notarized statement from your legal parent or legal guardian (e.g., on Form DS-3053) which states he or she supports you receiving a passport. The statement must be accompanied by a photocopy of that parent or guardian's ID.
Applicants Under Age 16:
Applicants under the age of 16, must demonstrate parental consent at the time of their appointment. Parental consent is most easily demonstrated the following way:
- Child and both parents/legal guardians present at the time of the application.
- Parent’s/Guardian’s drivers’ licenses.
- Child’s birth certificate.
If it is not possible for both parents/legal guardians to be present at the passport appointment, please review the Department of State’s website for alternative options to demonstrate parental consent.
Special Circumstances:
Proof of Name Change: If your name has changed for any reason (e.g. marriage, adoption, etc.), you will need to bring in original documentation (marriage certificate, adoption papers, etc.).
Was your most recent passport lost or stolen? You must report your passport as lost or stolen in order for a new passport to be issued.

Fees
At your appointment you will be required to pay two, separate fees. One to the US Department of State and one to the FSU Passport Acceptance Facility.
US Department of State Fee
The first fee is the required application fee for the U.S. Department of State, this fee is payable ONLY by check or money order.
Please ensure that the check or money order for the application fee is exact. If opting for 1 to 2 day shipping (see below), DO NOT round the total to the nearest dollar amount.
Additional, optional services offered by the US Department that can be included in your application fee are as follows:
- Expedited Processing: Available at an additional cost of $60, this expedites the processing time for your new passport, ensuring a quicker turnaround.
- 1 to 2 day shipping: Once your passport is printed and ready to be shipped it will be delivered within 1 to 2 days. This service only applies to addresses within the United States and is not valid for passport cards.

FSU Passport Acceptance Facility Fee
The second fee you are required to pay is the $35 execution fee, this fee is paid directly to Florida State University ONLY by credit/debit card. This fee is different from the application fee which is payable by only check or money order to the US Department of State.

Renewing a Passport
Renew Online:
Eligible U.S. citizens can now renew their US passports online using the online passport renewal system. Please keep in mind that online renewals are not eligible for expedited processing.
To be eligible for the online renewal, you must meet the following requirements:
- Your most recent passport was valid for 10 years, and you are 25 or older
- Your most recent passport was issued over 9 years ago, but less than 15 years from the date you submit your application
- You are not changing your name, gender, date of birth or place of birth
- You are not needing expedite processing
- You can upload a digital passport photo

Renew by Mail:
A person qualifies for renewal by mail if your most recent US passport meets the following criteria:
- Is undamaged and can be submitted with your application
- Was issued when you were age 16 or older
- Was issued within the last 15 years
- Was issued in your current name or you can legally document your name change
If you qualify for renewal by mail, our office CANNOT accept your application and you must mail it off on your own. We can, however, provide you with the appropriate renewal form. You may also obtain the necessary photo for your application from our office for $10.74. Please be advised that you must reserve an appointment to have photos taken. If you do not meet the above criteria, you must apply for a new passport in person.
Renew In Person:
For applicants who do not qualify for either online or mail-in renewals, you must apply for a new passport in person at a passport acceptance facility using Form DS-11. This pertains to individuals whose previously issued passport was:
- Issued over 15 years ago
- Issued under the age of 16
